This is the detailed one: it asks for owner identity including a Social Security number, and it takes 4 months of bank statements, photo ID and a voided check as uploads. Your answers save on this device as you go, so you can stop and come back on the same browser.

What this form collects

Business identity: legal name, EIN, address, entity type and start date
Trading detail: annual sales, card volume, premises and any existing advance
Owner detail: home address, date of birth, Social Security number and licence number
Documents: 4 months of bank statements, photo ID and a voided check
A signature, with the date, time and IP address it was made from

The application is assembled into a single signed PDF in memory, emailed to us, and then discarded. There is no database behind this site and no copy of your documents is left on the web server.
